We work closely with you and your providers to help you have more healthy days at home.

Somatus partners with health plans and providers to deliver customized kidney and cardiovascular condition support services to their eligible members at no extra cost — health plan members like you.

Our healthy living program provides:

  • One-on-one care – We offer health coaching and assistance 100% based on your individual needs.
  • We come to you – Whether it is your home, treatment center, or just a call on the phone, we support you when and where it’s easiest for you.
  • 24/7 assistance – Nurses are available all day every day to help with questions or concerns regarding your health.
  • Healthy eating tips – Our dietitians help with meal planning assistance based on your personal health and goals.
  • Community support – Somatus social workers and patient health advocates can link you to local resources that can help with meals, medications, transportation, and more.
  • No extra cost – Our program is covered by certain health insurance plans at no additional cost for eligible members. Find out if you’re eligible!

Care Team Member

How They Can Help

Nurse Care Manager

Nurses will watch over your care. They can help you with your treatment plan and answer health questions. They will check in to see how you feel.

Patient Health Advocate (PHA)

PHAs can help you learn what your health plan will cover. They can assist you with obtaining medical supplies, transportation services, financial assistance (food and medications), and more. They can also help you sort out what healthcare decisions matter to you, so you can fill out an Advance Directive.

Social Worker

Social workers can discuss your mental and behavioral health needs. They can help connect you to a behavioral health provider if needed and help you find local support groups.

Registered Dietitian (RD)

Registered dietitians will ask what foods you like and help you to incorporate them every day to support healthy eating. They can also help you meal plan and understand what foods are good for you and those that can negatively impact your condition.

Nurse Practitioner

Nurse practitioners will work with you to receive health screenings and wellness checks. They will also check in to see how you feel.

Pharmacist

Pharmacists can help you understand how your medicines work. They can also let you know if new medications will react badly with ones you’re already taking and if the dose needs to be adjusted for your health conditions.
